So, I've had my ef for about 2 years now. I did the MPFI on my d15b and had no problems. Now a year later, every time I start the car it's pretty rough. It takes a split second for it to turn over and when it doessss.. It idles around 3ish and pulses. It does this regardless if it's cold or warm. It also does a lil 'misfire' at idle sometimes. I guess after the car 'warms up' (if that's what it's doing), It runs fine. I cleaned out the IACV thinking that might be it, but nothing changed with that.
Any insight?

try doing cap and rotor,or testing your dizzy.
Last time you did a tuneup?

That could be one of the problems though.
You never know with hondas.

I believe those engines also have an FITV (Fast Idle Thermo Valve), if it does that's a good possibility for it being your problem. It's on the throttle body, affects cold start only (cold as in new start, not cold weather). Also may be worth checking for vacuum leaks.
Should fix your problem, but I can't remember 100% of that engine has one or not.

Tps? I don't have a fitv. And def not the starter lol. Last tune up was less than a year ago

Well I say dizzy. Try changing that or it could be the cap even. Is it cracked or anything? Change plugs and plug wires.

Was cap and rotor done? Also check like was said for intake leaks,as the gaskets will heat up and expand, but a cold start could give it problems.

The misfire at idle makes me think its a intake gasket. Take a can of carb clean and spray it all around the intake manifold, if you notice a change in the idle, or the car stalls out you know you found the problem.

Yeah idling around 3k at start up is usally the FICV, or possibly a bad gasket somewhere. Although I would imagine that if you had a bad gasket even when warmed up it still would idle that high.

I know someone said dizzy earlier, that's not likely. That problem would be persistent and would be more likely to occur when the dizzy is hot.
TPS problem would more than likely cause issues all of the time. Lots of stuttering and bucking probably at tip in throttle (right when you first get on the gas especially lightly during cruising).
Being as there is no FITV I'm gonna go with what Honduh said, you probably have a fairly large vacuum leak somewhere around your intake manifold, post MAP sensor. Shouldn't be too hard to find, a can of carb cleaner or just putting on the bubbles from a soapy lather. It'll suck them right in.
